Kid's Church Family Registration

Welcome to Christ Chapel Kid's Church, for ages 1-5th grade! Completing this form will help us to serve you & your family better, and allow you to Check-In to Kids Church & other events! Please check in at our Check-In Station Kiosks every week that your kids plan to attend Kids Church.

If you are registering in the summer and your child is between grades, please enter the grade your child most recently completed.

Please keep your phone near you while your kids are in Kids Church.

We will use your phone number to contact you in case you are needed while your child is in Kids Church. With little ones, we have a 10 minute cry policy. We will do all we can to console your child, but if they are still crying after 10 minutes we will send a message to your phone and ask you to come to the infant & toddler area. We feel this will best serve you and your little one. You are welcome to stay in the Kids Church area with your child for the remainder of the service if you choose.

We occasionally take photos and videos during Kids Church and related activities. Do you grant permission to use such photographs and videos of your child/children taken during Kids Church and other related activities for public use on, but not limited to, print and digital media formats including print publications, our church website, posters, banners, flyers, and social media?

Once this form is submitted, your child/ren will be officially registered! You can check in at the kiosks using the phone number you provided above.
